ABOUT US
The Community Transformers Foundation is a 501(c)(3) nonprofit organization serving communities in the Commonwealth of Virginia. The organization seeks to advance health and social equity by addressing the social determinants of health through safety net programs and community partnerships. CTF is governed by a board of directors from some of the brightest minds in the Commonwealth of Virginia. These board members are experts in the field of healthcare, housing, food insecurity, education, financial literacy, and social justice. CTF Corporate offices are in Hopewell, Virginia with a new satellite office opening soon in South Hampton Roads

Our staff consist of some of the most respected professionals in the area who possess the knowledge and experience to build coalitions that will greatly benefit the community and those who live within. CTF’s mission is to be an organization that is dedicated to building collaborations within communities to develop sustainable programs that address the social determinants of health and promote health equity.

Our Values
Build up Underserved Communities by Promoting Health and Social Equity
A healthy community is crucial to achieving successful outcomes, whether social or economic. The Community Transformers Foundation addresses health gaps in communities where access to quality healthcare is limited, and connects communities with a network of healthcare professionals who can provide screenings, counseling, and education so that people will have the information they need to make the best decisions regarding their health and wellness needs
Our Strategy
Leveraging Knowledge, Skill and Innovation
CTF’s strategy is to partner with community leaders, community-based organizations and residents to develop sustainable programs that will ameliorate issues around health inequity and social determinates of health. CTFs board of directors were chosen specifically for their knowledge, relationships to leaders (either as one or connection to) and their passion for empowering communities. CTF builds collaborations with the communities they serve to design and implement programs to promote healthy outcomes.
