About the Event
The Community Transformers Foundations Strategic Plan is to advance health and social equity in Virginia. The Foundation seeks to build the capacity of the organization through a combination of community programming, partnerships and safety net programs. The Foundation focus is on the following areas: Economic Stability – Poverty, Employment, Food Sustainability and Housing Stability Social and Community – Context, Social Cohesion, Civic Participation, Perceptions of Discrimination & Equity and Incarceration / Institutionalization · Education – High School Graduation, Enrollment in Higher Education, Language & Literacy and Early Childhood Education and Development Health and Healthcare – Access to Health Care and Primary Care, Health Literacy, Quality of Housing, Crime & Violence and Environmental Concerns
