Transport Connect

The Trans Connect program helps to improve the lives of the elderly and vulnerable populations by delivering food and medical supplies to people who live in rural areas and food deserts. The Trans Connect program is for individuals who are impacted by a lack of resources in addressing food insecurity and medical needs such as care, medications, and medical equipment. We believe providing transportation delivery services for food and medication is essential to these communities.

How does our program work? We work with local community partners to identify individuals
with chronic transportation needs around food and medical challenges:

● Local Pharmacies
● Physician Offices
● Food Bank/Pantries
● Local Department of Social Services
● Area Agencies on Aging
● Senior Connections
● Faith Based Institutions
● Local Law Enforcement Agencies

We offer telephone support and online registration through our website for individuals to sign up
for delivery services.
